Merton Home Treatment Team

We provide crisis and home treatment support to Adults aged 18 - 75 years of age who live in Merton and are experiencing a Mental Health Crisis and have increased risks.

24 hours

You will see various professionals in the team consisting of medical and nursing staff, Recovery Support Workers, Social Workers, Peer Support Workers, Occupational Therapists and sometimes medical and nursing students on training placements.

When you are referred to our service, we will offer you an Initial Assessment of your mental health needs and agree a plan of care with you, to support you during the Crisis.

The team can see you initially on a daily basis to provide support to you and your family. Depending on your progress we will adjust the frequency of the visits whilst gradually working towards a discharge plan.

We will also offer ongoing assessments to help you through the crisis and stay out of hospital. We will work with you right through to the point of discharge, during which we will work with you to develop a holistic plan of care and support (collaborative crisis plan). We can also refer you to appropriate services for follow up care and support after the crisis.

You can be referred to us via different agencies, namely your G.P (if out of hours), Merton Adult Assessment Team, Accident & Emergency Departments, Mental Health Community Teams and other emergency services. Please email MertonHomeTreatmentGroup@swlstg.nhs.uk if you have any questions.
