Merton Memory Assessment Service

Who we are


This service assesses adults with memory problems to detect if they have the symptoms of dementia. Early diagnosis enables people to take advantage of new treatments to slow the onset of the condition, to plan their care and maintain a higher quality of life.

This service is for adults with memory impairment  who live in Merton.

Monday-Friday 9am - 5pm

What you can expect


You will be offered an appointment with a Health Care Professional at one of our outpatient clinics, or be given a home visit in exceptional circumstances. You will have an assessment and possible referral for investigations and given diagnostic feedback and short term follow up.



Referrals should come from GPs. Please email if you have any questions.
