Merton Older People's Community Mental Health Team (CMHT)

Who we are

This service is for people over the age of 65 with dementia or other organic illness and over the age of 75 with functional mental illness such as depression.

Who is the service for

We are a multi-disciplinary team member which is made of nurses, consultants, other medical staff, occupational therapists, and psychologists.

What you can expect

When you use our services you will receive an assessment of your mental health needs and a care plan detailing the support you will receive. Everybody who uses our services has one named person who will co-ordinate their care and support.  This person will be either a named professional or, if your needs are more complex, a care co-ordinator. This person will be your main point of contact and you can talk with them about how your treatment and support are going.

Referrals

Referrals to the team can be made via email MertonoPAdminTeam@swlstg.nhs.uk or telephone.  Referrals can be accepted from:

  • General Practitioners whose patients live within the Trust localities.
  • Health professionals providing services at (local district) Hospital
  • Liaison Psychiatry Department
  • Duty Psychiatrist and Duty Psychiatrist for A and E
  • Social Services Area Teams
  • Emergency Duty Social Work Team
  • Local Adult CMHTs
