Merton Older People's Community Mental Health Team (CMHT)

Who we are

This service is for people over the age of 65 with dementia or other organic illness and over the age of 75 with functional mental illness such as depression.

Who is the service for

CMHT is for people aged over 65 suffering from severe mental illness and behavioural and psychological symptoms of dementia (BPSD).
The Home Treatment Team (HTT) is for those people who are on the CMHT team caseload who require short term intensive support at home to help to manage severe mental health difficulties and to avid admission to hospital.

What you can expect

If you use our services you will receive an assessment of your mental health needs and a care plan detailing the suport you will recieve. Everybody who uses our services has one named person who will co-ordinate their care and support.  This person will be called either a named professional or, if your needs are more complex, a care co-ordinator. This person will be your main point of contact and you can talk with them about how your treatment and support are going. 


Referrals to the team can be made by letter or telephone contact by previous carer or user of the service.

Referrals can be accepted from:

  • · General Practitioners whose patients live within the Trust localities.
  • · Health professionals providing services at (local district) Hospital
  • · Liaison Psychiatry Department
  • · Duty Psychiatrist and Duty Psychiatrist for A and E
  • · Social Services Area Teams
  • · Emergency Duty Social Work Team
  • · Self and carer referral of recent users of the service
  • · Local Adult CMHTs.
