What you can expect
Anyone who uses our services first gets an assessment of their mental health needs and a plan of the care and support they will receive. They will also will also have one named person who co-ordinates their care and support. This person will be called either a named professional or, if your needs are more complex, a care co-ordinator. This person should be your main point of contact. You can speak to them about how your treatment and support are going.
If you are referred into the Mitcham Recovery and Support Team this is usually done, with your agreement, through Merton Assessment Team which is based with us at The Wilson Centre.
All statutory and voluntary agencies could refer to the service. Family/Carers and self-referrals is also accepted. We also receive referrals from other assessment team, Home Treatment Team, Inpatient Service, Forensic Team and Early intervention Team and others.
When you are referred into the service your details will be passed to us. We will contact you to get some more information and discuss what the next steps available to you are.
