Referrals
The e-RS route is now the only way for GP Practices to send in referrals. This means that all first outpatient referrals must come into our Trust via the Electronic Referral Service (e-RS).
Written referrals should come from a consultant psychiatrist and should include presenting problems, past psychiatric history, prior treatment, medication history, current care plan and the appropriate forms. Please send all your referrals forms to adminocdbdd@swlstg.nhs.uk
- Basic Requirements form 1
- Inpatient Admission Requirements form 2
