Who we are

This is a Community Mental Health Team (CMHT) for people with mental health problems in the Putney and Roehampton areas.   It provides care and treatment.

Who is the service for

This service is for adults aged 18 -75 who live in Wandsworth or have a Wandsworth GP

What you can expect

 

Anyone who uses our services receives a plan of the care and support they will receive. They will also will also have one named person who co-ordinates their care and support. This person will be called either a named professional or,  if your needs are more complex,  a care co-ordinator. This person should be your main point of contact.  You can speak to them about how your treatment and support are going.

 

How to contact us

Referrals

Your GP should always be your first point of contact for any mental health related issues.

Your doctor can then refer to the Single Point of Access (SPA) if appropriate. Referrals are usually via letter or efax to the SPA team.
