The Richmond Older People’s Team comprises of the Richmond Memory Assessment Service, Richmond Intensive Outreach Team, Richmond Care Home Liaison Service and Richmond Recovery and Support Team.

Richmond Memory services

The memory service is designed to meet the needs of adults of all ages with symptoms of mild to moderate dementia who have not already received a diagnosis. General practitioners refer patients who they suspect may have mild to moderate dementia which is beginning to impact on the person’s day-to-day living/people with memory problems, where a diagnosis of dementia is suspected; people with dementia, where confirmation of the sub type and/or specialist advice is required

Richmond Intensive Outreach Team

The Richmond Older People Intensive Outreach Team provides care for people who would normally require hospital admission and stay involved until the client is able to be managed by the OP Recovery and Support Team. They provide intensive, home based treatment on a seven day week basis for those requiring acute care. The working hours are Monday to Sunday 9am to 5pm.
