Who we are
We provide service for residents living in Sutton Borough who are 18 years old or over and have formal diagnosis of Learning Disability or Intellectual Disability who need support with Mental Health needs.

What you can expect
Anyone who uses our services first gets an assessment of their mental health needs and a plan of the care and support they will receive. They will also will also have one named person who co-ordinates their care and support. This person will be called either a named professional or, if your needs are more complex, a care co-ordinator. This person should be your main point of contact. You can speak to them about how your treament and support are going.

Referrals
*From May 2022 the e-RS route will be the only way for GP Practices to send in referrals. This means that all first outpatient referrals must come into our Trust via the Electronic Referral Service (e-RS).
