Who we are
Sutton Home Treatment Team (HTT) offers both an alternative to admission and facilitates early discharge. We also offer crisis intervention.

This service is for adults aged 18-75 who live in Sutton.

What you can expect
Anyone who uses our services first gets an assessment of their mental health needs and a plan of the care and support they will receive. They will also have one named person who co-ordinates their care and support. This person will be called either a named professional or, if your needs are more complex, a care co-ordinator. This person should be your main point of contact. You can speak to them about how your treatment and support is going.
How to contact us
Referrals
Patients are referred through: any secondary mental health care teams, A&E departments, Sutton Uplift/Sutton Assessment Team. We do not accept self referrals, GPs should contact Sutton Uplift for referrals. Patients have to be a permanent or temporary resident living in the borough of Sutton during their stay with us.
Referrals are accepted by phone only, but we kindly request you follow instructions on this short Sutton HTT Referral Form and send it to us before calling the number listed on the form, so you can prepare for what we might ask, and to reduce communication and handover errors.
