Sutton Uplift

Who we are

Sutton Uplift is an integrated community based mental health and wellbeing hub which is accessible to anyone over the age of 18 living in the borough of Sutton or registered with a Sutton GP. We offer services to those with common mental health problems or with severe mental illnesses such as psychosis, bipolar disorder and to those with personality disorders.

Sutton Uplift is a partnership between the NHS and local voluntary sector organisations focusing holistically on supporting your mental, physical and social needs. Our teams work closely with GPs, social services, the voluntary sector and other organisations to offer personalised treatment and care in a more joined up way.

Who is the service for

This service is for anyone aged 18 and over, living in Sutton or with a Sutton GP.

Referrals

The e-RS route is now the only way for GP Practices to send in referrals. This means that all first outpatient referrals must come into our Trust via the Electronic Referral Service (e-RS).   

You can refer yourself to the service online or by telephone, or you can be referred by your GP, a carer, a family member or someone else such as social services. You or the referrer will need to complete a short questionnaire to gather key information.
