We plan the form of treatment and care that will aid those we support so that they will recovery from their mental health difficulties as much as possible. This does involve aiding people to access other services such as the local authority or voluntary sector agencies who could meet people’s identified needs. When people have an open case to our Team they will have one named person who co-ordinates their care and support. This person will be called either a named professional or, if your needs are more complex, a care co-ordinator. This person should be your main point of contact. You can speak to them about how your treatment and support is going.
